A volunteer service that supports vulnerable patients at home on the day of their discharge from hospital in order to reduce readmissions up to 72 hours post discharge. Volunteers meet patients and ensure that they have food and drink, and a safe environment at home (e.g. the lighting, water and heating are all fully functional).

This guide uses learning from the "Settle in Service" being delivered by the Norfolk & Norwich University Hospitals. The purpose of the guide is to provide a potential service adopter with the information needed to be able to:

a) Decide if the service would be of benefit to their organisation

b) Understand what considerations are needed to adapt the service to their environment
