Falls prevention volunteer role profile and referral form

As promised in our previous post, we will regularly share with you key documentation developed by Kingston Hospital throughout the implementation of the falls prevention volunteer project.

Last week the community exercise volunteer role went out to advert and here is the finalised role profile.

In addition please see the referral form which ward staff will use to refer patients to the service. This form may be further developed as the project evolves, when we go live we will review all shared resources and collate final versions into a single pack for ease of access.

In the next couple of weeks we will share the evaluation plan which has been developed using the Helpforce I&I service.
